I understand that participation in tennis activities — including lessons, training sessions, organized play, tournaments, and related events — involves inherent risks of injury. These risks may include slips, falls, collisions, equipment-related injuries, weather conditions, and other unforeseen hazards.

I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury that may occur as a result of my participation.

In consideration of being permitted to participate in activities organized by Aces Club Inc., I release, waive, discharge, and covenant not to sue:

• Aces Club Inc. and its officers, directors, board members, employees, volunteers, instructors, agents, and representatives
• facility owners and operators (the facility) and its board members, officers, employees, management, agents, and representatives

…from any and all liability, claims, demands, actions, or causes of action arising out of or related to participation in tennis activities conducted at the facility facilities or any other location used by Aces Club Inc.

I agree to indemnify and hold harmless Aces Club Inc., its contracted Lead Instructor(s), and the owners or operators of any facility used by Aces Club Inc., from any loss, liability, damage, or costs (including reasonable attorneys' fees) arising from my participation in activities, instruction sessions, or tournaments, or from my violation of facility rules or policies.

I agree to:

• Follow all facility owners and operators rules and regulations regarding court use
• Respect residents and shared amenities
• Follow instructions from designated Aces Club organizers
• Maintain sportsmanlike and respectful conduct at all times

Failure to comply may result in removal from participation.

I affirm that I am physically capable of participating in tennis activities. I understand that Aces Club Inc. does not provide medical insurance coverage for participants. I am solely responsible for any medical expenses incurred as a result of participation at any location where Aces Club Inc. conducts activities.

I grant Aces Club Inc. permission to use photographs or video taken during activities for nonprofit and promotional purposes.

I have carefully read this agreement, fully understand its terms, and understand that I am giving up substantial legal rights by signing it. I sign this agreement freely and voluntarily.
